Scheduling appointments
Call our office during business hours at (603) 898-2244, or schedule a follow-up appointment as you leave our office.
Canceling an appointment If you must cancel an office appointment, please call at least 24 hours in advance, so that we may accommodate another patient.
Office hours 9 a.m. - 5 p.m. Monday – Friday
When you come for your visit Please bring any medical records, lab results, x-rays or other medical imaging related to the condition for which you are seeking treatment. Please note that for imaging studies including x-rays, CT scans and MRIs we need to see the actual images, not just the report. You should arrange to bring a CD or a copy of the films to the office.
To make sure that we have copies of all pertinent information at the time of your visit, make sure you have a copy of the information with you. When you ask another health provider's office to forward information to us, we may not receive it in time for your visit.
Please have your insurance information available when you come for your first visit or whenever there is a change in your insurance information . If your insurance requires that you have a referral form from your primary care physician, you must present it at your appointment. If you do not have this form, your evaluation may be delayed, and you may be asked to reschedule your appointment.
