Instructions for New and Returning Patients

Before you come to the office

• Make sure you have a list of all your past medical problems, surgeries, medications and allergies.
• If you had x-rays, an MRI or a CT scan, make sure you bring the actual images with you on films or a CD along with the written radiologist report. The report alone is usually not helpful.
• If we do not have access to x-ray images, we may obtain another set in our office.
• If you had an EMG or a nerve test, bring the results with you.
• For all tests, the only way to ensure they are available for review at the time of your visit is if you bring them in with you to the office. You can never count on having another office sending the appropriate results in time.

Shoulder Problems

• For women, your shoulder can be more easily examined if you are wearing a tank top.
  

Hip or Knee Problems

• Your hip or knee is most easily examined if you are wearing shorts.
